Nursing Home Care / Assisted Living Program

 Maine Medical Center Geriatric Physicians, Physician Assistants and Nurse Practitioners are available to provide ongoing, primary patient care at area nursing homes and assisted living facilities, including Maine Veterans Home and Piper Shores in Scarborough, Seaside in Portland, Ledgeview Assisted Living Facility in Cumberland, Falmouth by the Sea in Falmouth and Freeport Nursing and Rehabilitation Center in Freeport. Maine Medical Center physicians provide medical directorship at some of these sites as well. These facilities also provide an important educational experience for Geriatric Fellows and other Internal Medicine and Family Medicine residents doing rotations in Geriatrics. The presence of MMC physicians in long term care settings allows for a smooth transition for those elders moving from an acute hospitalization to a skilled nursing facility. 

In addition to nursing home care and assisted living care, our geriatricians are also available at New England Rehabilitation Hospital of Portland and at Hospice of Southern Maine.
